The purpose of this assignment is for you to describe the best scenario for adolescent and young adults in your patient population and to think through what tasks would need to happen for that scenario to be realized.

Your assignment is to create an Action Plan, which allows you to break down this optimal scenario into smaller tasks, some of which may be achievable in the short- or long-term. The Action Plan also requires you to think about who would be responsible for completing the task and what resources would be needed. The most effective Action Plans are also measurable. It is important that you think about how you will know that the Action Plan has been successful.

Once you have created your Action Plan, you will have the foundation to implement it into your workplace. This is not a requirement, but rather a suggestion for leveraging the work you’ve done on this assignment.
